I want to create a response form on a html document. I use dreamweaver to create the form.

I have tried this before without any success.

If someone could please refer me to some easy to follow instructutions,
I would be gratefull.

Creating the form's easy, what do you want to do with the data you collect ?

You'll have to post it to a script and then the script can email / populate database etc...

Alex,
I feel that I can create the form.
The form may have 40 fields to be filled in, but I want
the form to come to me through email or another method if
available.

If it comes email, I need help understanding about the script.
What script, how to edit, where to put it, and how to get it to
come to my in box.

Your hosting provider possibly has a script called FormMail which could do this, ask them what they have on offer.

It's a fairly basic bit of PHP if you want to do it yourself.

I can download FormMail from my control panel.
But my host really does not offer support for installing
software programs.

I will install FormMail and give it a try.
